How Do I Apply for Medicaid in Georgia?


Medicaid is a state-operated, federally and state-funded health insurance program for low-income elderly or disabled individuals, pregnant women, children and, in some cases, special populations such as the HIV positive, women with cervical cancer and refugees. The Georgia Division of Family and Children Services manages the Georgia Medicaid program.


Instructions


1. Gather documents and information necessary for completing the Medicaid application. You will need a state-, federally or military-issued photo identification card; birth certificate; Social Security card; documentation of all sources of income and resources; and if applying for Medicaid for the disabled, the name, address and phone number for all medical providers.


2. Fill out the Medicaid application. You will need to write in the full name, date of birth and Social Security number for all household members, income and resource information and other related information.


3. Locate your regional office (see resources) and submit the application and all supporting documentation either via mail, in person or by fax. You will receive a response within 10 to 60 days, depending on the type of Medicaid you are applying for, stating approval or denial. If denied, proceed to step 4.


4. Compose an appeal letter that states your full name, date of birth and Social Security number that states that you are requesting an appeal of the decision. Provide specific reasons for the appeal. For example, if you were determined to not be disabled, state that you are filing an appeal on the basis that you believe you do have a qualifying disability.


5. Gather documentation, such as medical records and statements from previous employers, as evidence for your appeal case. Submit this documentation to your regional office.


6. Attend an appeal hearing with an administrative law judge. You may have an attorney, family member, mental health advocate or another individual represent you during this hearing. You will receive a response within 10 to 60 days after the hearing.
