Write A Medical Disclaimer For An Ultrasound

Ultrasounds can help doctors determine whether surgery is necessary.


Medical technology is advancing virtually on a daily basis. This allows doctors and nurses to use sophisticated machines in order to diagnose medical conditions. A machine that medical professionals use frequently is the ultrasound. This machine uses sound waves to create images of internal organs and tissues in a patient's body, and is considered safer than some alternative machines such as X-rays. Like any medical equipment, medical ultrasounds require disclaimers that need to be written properly to avoid legal issues.


Instructions


1. Place your hospital, clinic, or manufacturer logo at the top of the disclaimer document. Center or left justify this and place the contact information for the hospital, clinic, or manufacturer underneath the logo.


2. Write a label for the disclaimer, such as "Disclaimer" or "Medical Ultrasound Disclaimer and Consent Form." Place this in bold font, centered and several lines down from the logo and contact information.


3. Write a paragraph that describes how the ultrasound is intended to be used and of what it is capable (e.g., Ultrasound Model Such and Such is capable of producing images in 3D and, therefore, is suitable for diagnosing conditions such as X, Y, and Z).


4. Write a paragraph that explains the limitations of the ultrasound. This should include a statement about the possible likelihood of error in the testing results due to the fact that the images produced are interpreted by individuals. It also should include some examples of other factors that may impact results, such as excess gas or placement of internal medical devices.


5. Write a paragraph that explains what liability you accept or disclaim. For example, you may wish to state that you are not liable for procedures done unnecessarily due to incorrect use of the machine or poor interpretation of the images.


6. Write a paragraph or bulleted list that explains the cost of using the machine and what is included in the ultrasound test. Include a statement in which the patient agrees to cover the costs not covered by insurance.


7. Include a final paragraph stating that the medical risks of using the ultrasound have been explained to the patient.


8. Provide lines, clearly labeled, on which the patient can write the name of their physician, health insurance information, signature, and date.
