Create a fillable PDF form using Adobe Acrobat.
Portable Document Format (PDF) is commonly used to create interactive forms, such as electronic applications or surveys, in which recipients can fill in information without printing anything on paper. To make a PDF fillable form, you need the Adobe Acrobat full version software. You can start a form from scratch, convert another electronic document to a PDF form or convert a paper form. If you're starting from a blank page, it may be most efficient to type and lay out the form using a word processor first and then convert it.
Instructions
1. Open a blank document in a word processor. Begin typing the content you want to include in your PDF form. Format the text as needed, making desired changes to font, font size and layout of the document.
2. Save the file. Make sure to edit the content as much as possible before converting to PDF. It will be much more difficult to edit after you've already made the document into a form.
3. Open Adobe Acrobat.
4. Select the Forms menu and choose Start Form Wizard. Select An Existing Electronic Document. Choose the file you just created and follow the on-screen prompts to convert it to a PDF form.
5. Select the Forms menu again. Choose Add or Edit Fields.
6. Click Form Tools under the Forms menu to begin adding interactive form fields, where recipients will be able to fill in information. Select a form tool that you want to use to create the first form field. Each form tool creates a different type of form field, such as a check box, text field or button. The cursor will become a crosshair.
7. Click an area on the page where you want to enter a new field. Click and drag to draw a rectangle. This will define the size of the form field you are creating.
8. Enter a name for the field in the Field Name box. This will identify each form field as you organize the data.
9. Repeat steps 6 to 8 for each form field you need.
10. Click File, Save to save the form.
11. Click the Preview button to test out your form as it will be seen by recipients.
