The state of Kentucky offers Medicaid services to ensure that low-income families have access to medical insurance. In order to apply for Kentucky Medicaid, you must have children in the home under 18 years of age, be pregnant, elderly, blind or disabled. If a child is in your home is 19 years old, you can still apply for Medicaid if the child is in high school, vocational school or expected to graduate during the month of her 19th birthday. Unlike many states, Kentucky does not allow you to apply for Medicaid services online.
Instructions
1. Ensure that you meet the income eligibility requirements. The income limit is $217 per month per individual in the household. The resource (assets) limit is $2000. For each additional person living in your household, add $70 to the income limit.
2. Visit your local Department For Community Based Services (DCBS). If you need to find an address for your nearest DCBS office, use the "Local Office Search" tool that is located on the Kentucky Cabinet For Health & Family Services website.
3. Request an application for Kentucky Medicaid. Complete the Medicaid application.
4. Present the completed application to the DCBS representative. Along with the application, submit a copy of your Social Security card, driver's license or state issued photo ID, birth certificate and proof of income, bank statements and medical records to prove pregnancy (if applicable). Proof of income can include check stubs, tax returns and 1099 forms.
