Generally, birth certificate changes can be made at the hospital within a certain period.
It is important to note that birth certificates data and records are maintained and distributed within local municipalities (state, town, city or county vital records or statistics departments). While this information may vary between different municipalities, many aspects of the process will be similar between different areas. In New York City, corrections can be submitted through the hospital where the person named on the birth certificate was born up to a year after birth, at no cost. In general, the request for corrections must be made by the person named on the certificate (if they are over the age of 18 in most instances) or the parent or parents of the person named on the certificate.
Instructions
1. Make changes directly with the hospital where the person named on the certificate was born, in some muncipalities. Find the medical records, health information or hospital registrar to process changes. Bring the original birth certificate and current, signed photo identification, such as a driver's license, non-driver's license, passport or military ID card. Wait for at least a week for changes to be made and in general, expect that these changes can be made at no cost.
2. Locate the website or office of the vital records department within the town or county where the person named on the birth certificate was born. Procure an application for making changes and learn about the supplemental materials needed in addition to your application, like signed and current forms of photo identification, an original birth certificate document (either with official letterhead or with an original seal as issued by the birth certificate granting department), as well as an additional photocopy of the document. Contact representatives of the office in order to verify which steps need to be taken in accordance with the name changes you desire to make, as this process varies between municipalities across the country.
3. Expect to pay a nominal fee for processing and/or an additional fee for a new copy of the certificate. Be prepared to wait up to several weeks for processing revisions and receiving an updated copy of the birth certificate. Submitting these forms by mail may require you to include a self-addressed stamped envelope.
