Replace A Lost Birth Certificate In Indiana

Apply for a replacement Indiana birth certificate by mail or in person.


The Indiana State Department of Health makes it easy to replace your lost Indiana birth certificate. To receive a certified copy, provide proof of identification for yourself or for the person whose name is on the birth certificate. The cost varies if you visit a county office of vital records, but is $10.00 for the first copy if you order it by mail or go to the Indianapolis vital records office.


Instructions


Identification


1. Apply for a birth certificate for yourself with one of these proofs of identification: driver's license, current work identification, state identification, current school identification, veterans card, military identification with signature, corrections identification, passport, Mexico consular identification, probation identification, verification of employment written on the company letterhead, or a referral from a state agency.


2. Apply for a birth certificate for someone else by establishing proof of your relationship. For a parent's birth certificate, you need your own birth certificate, a grandparent's requires your identification and mother's or father's birth certificate. To get your spouse's certificate, you need your marriage certificate and your identification; for child's or grandchild's, supply your identification and your birth certificate. Other relationships require different documents (see Resources).


3. Make copies of the requested identification requested if you're ordering the certificate through the mail. If you're making the request in person, you can supply either copies or the actual documents.


By Mail


4. Print and complete State Form 49607 (see Resources).


5. Write a check or buy a money order payable to the Indiana State Department of Health. The first copy of a birth certificate costs $10.00; additional copies of your certificate are $4.00 each.


6. Mail the form and payment to:

Include your return address in the upper-left corner of the envelope. You will receive your replacement certificate in about a month, according to ISDH.


In Person


7. Go to the Indiana State Department of Health Vital Records Office. The office is open from 9 a.m. to 2 p.m. Tuesday through Friday, and is closed on state holidays. The wait is usually less than an hour, according to the ISDH. You can also go to the Vital Records department of the county in which the person needing a birth certificate was born (see Resources).

8. Request a copy of your lost birth certificate and show your identification.


9. Pay the fee with cash, a check or a money order. At the head office, the first birth certificate costs $10.00; each additional copy is $4.00, according to the Indiana State Department of Health. At other offices, the cost varies from $5.00 to $15.00 for the first copy.
